Well!

Any episode is a good episode if Gaius is tempted out on location - and gets there (and presumably back) without being slung over someone else's horse! :-)

♦ The usual suspects this week, though perhaps not Puzzlewood...? Unless that's where they all paused to have a look at Lamia's ruined castle-haunt. But then anywhere within driving distance of Cardiff might have a mossy tree-root like that.
♦ Of course we had much use made of Cosmeston Medieval Village.
♦ And there were various lovely forest scenes which we'll probably never identify for sure.
♦ I am pretty certain that when the knights entered Lamia's castle, it was through the largest entrance of Three Bears Caves, cunningly disguised to serve as somewhere other than the entrance to the Tunnels of Andor.
♦ Then a brief use of Clearwell Caves for the tunnels.
♦ And I am pretty sure the scenes towards the end inside the castle were all sets.

ETA: All the forest scenes, including with the mossy tree root, were filmed at Fforest Fawr!

Mr P and I totally agree about Three Bears Cave, we recognised it, trees in the background will match yours and our photos.
I want to look at some of the ride by scenes and compare with some of my photos, if I spot anything I'll be back.
